Who is the service for?
The service is for children and young people (from the age of 0 to 25) who have severe and profound learning disabilities, and for families of children and young people who have severe and profound learning disabilities. We have a team within the service who can support children and young people (up to 18 years old) with their physical and mental health needs.
What does the service do overall?
Sandwell Integrated Support Service is made up of six teams called:
- Children with Disabilities Team
- Transition Team
- Counselling Service
- Early Years Integrated Support Service
- Sensory Support Team
- Community Nursing and Psychology Service.
Each of these teams has different ways to support people with different needs or at different times in their lives. The service in which our staff work is the Community Nursing and Psychology Service.
How do we help?
Our service, the Community Nursing and Psychology Service, supports children and young people who have severe and profound learning disabilities. We support these young people with their physical and mental health needs. We can give support and advice to people who have physical health needs such as epilepsy or sleeping difficulties for instance. We can give someone support and advice if they are having emotional difficulties. We can also give training and advice to schools, professionals, and carers.
We have different kinds of staff working in the team so that we can give this wide range of support. We have one full-time and one part-time clinical psychologist, and one full-time and one part-time community learning disability nurse.
Who can refer to us?
Anyone who lives in the Borough of Sandwell or who attends a Sandwell school can refer to us. Many of our referrals come from families themselves. Sometimes another professional (a doctor, health visitor, or teacher, for example), may tell families about the support they can get from the service. We prefer referrals in writing with as much information as possible.
What about carers and families?
We liaise closely with parents and carers at every stage of our support.
How do you reach us?
Our working hours are 9am to 5pm, Monday to Friday.
