
The Summary Care Record is an electronic copy of key information from the record held at your GP Practice which will enable Trust staff to access SCR of patients that are involved in their care, unless patients have previously opted out of having an SCR.
SCR contains important information from GP records including medications, allergies and any bad reactions to medicines.
Patients will be asked if they give consent and are happy for staff to access their SCR to obtain relevant information to aid any care and treatment provided.
Any patients who do not wish their SCR to be viewed need to notice the professionals directly involved in their care. A note will be recorded on the BCPFT system and relevant paper records to ensure this request is fulfilled.
If a patient is unable to give consent, their care professional may access information if they feel it is at the patient’s best interests.
