Who is our service for?This service is for older people going through severe mental health crises who can benefit from treatment at home rather than in hospital.
How do we help?The home treatment team provides specialist integrated mental health support for older adults with complex mental health needs. We work with individuals whose condition has reached a critical point and where intensive involvement from a multi-disciplinary team will help to resolve or avert a crisis.
We aim to promote faster recovery from illness and prevent unnecessary hospital admissions.
We also support timely discharge from hospital and maximise independent living promoting patients to their optimum level of function.
Who can refer into our service?
Referrals are accepted from Consultant Old Age Psychiatrists, Community Mental Health Teams, the Older People’s Liaison Team, Crisis Home Treatment and Inpatient services.
What about carers and families?
Part of our role is to support carers. Where there is an acute mental health issue, we offer carer support as part of the treatment plan. Where someone’s mental health is stable and the carer is in need of continued support, we can help the carer get in touch with the specialist carer support team for older adults
